You can come to DocLine for care programmes if you have a chronic condition. A care programme consists of examination (diagnosis(, treatment, and guidance for one specific condition or issue. We set strict quality requirements for our care programmes, in compliance with the standards of the Society of Dutch General Practitioners (Nederlands Huisartsen Genootschap, NHG). Our care programmes are also organised electronically as much as possible.

care programme asthma and copd
In the Asthma and COPD care programme, the most important goal is the avoidance of (further) damage to the lungs. Through check-ups, we monitor your issues and may instruct a pulmonary function test to be performed.
care programme diabetes
If you have diabetes, we want to prevent that your sugar level causes damage to your body. This care programme includes several check-ups, as well as additional examinations or tests each year. That way, we keep your diabetes and blood sugar levels under control.
care programme cardiovascular disorders
The cardiovascular disorders care programme, also referred to as Cardiovascular Risk Management (CVRM), was created for people with chronic cardiovascular disorders. Are you at increased risk of cardiovascular issues? Then you’re eligible to participate in this programme. Together with the GP, you’ll look at where the risks are and what lifestyle or medication changes may be needed, to best manage the risk of cardiovascular complications.
